Assessment interview checkpoint¶
Use this file to resume an interrupted assessment without losing confirmed answers. Store it with the report under the same audience and sharing expectations. Keep notes concise; omit secrets, private personnel records, and unnecessary identifying details. Evidence references are optional and must be safe to share with the intended audience.
Assessment context¶
| Field | Value |
|---|---|
| Spec version | {{SPEC_VERSION}} |
| Profile name and version | {{PROFILE_AND_VERSION_OR_CUSTOM}} |
| Selected categories and exact items | {{SELECTED_SCOPE}} |
| System boundary and environment | {{BOUNDARY_AND_ENVIRONMENT}} |
| Assessment date | {{DATE}} |
| Evidence date or range | {{EVIDENCE_DATE_OR_RANGE}} |
| Intended audience / sharing | {{AUDIENCE_AND_STORAGE_OR_SHARING_EXPECTATIONS}} |
| Last updated | {{TIMESTAMP_OR_DATE}} |
| Next unanswered item | {{ITEM_ID_AND_TITLE_OR_COMPLETE}} |
For a custom profile, record the included item IDs explicitly. A profile version identifies the selection definition; it does not replace the assessment spec version.
Item answers¶
Include every in-scope item reached so far. Record importance and state independently because either may be confirmed while the other remains unresolved. Use Confirmed, Unknown, Deferred, Unconfirmed, N/A, or Skipped for each field's status. A confirmed value must include the participant-confirmed value; an unresolved value stays blank. Record a reason for N/A or Skipped.
| Item ID and title | Importance value | Importance status | State value | State status | Notes and rationale | Safe evidence reference (optional) |
|---|---|---|---|---|---|---|
| {{ID_AND_TITLE}} | {{1-3_OR_N/A_OR_BLANK}} | {{STATUS}} | {{0-4_OR_BLANK}} | {{STATUS}} | {{CONCISE_NOTES_OR_REASON}} | {{SAFE_REFERENCE_OR_NONE}} |
Resume instructions¶
Verify the spec version, profile, and selected scope with the participant. Carry forward confirmed fields and notes without asking for them again. Continue at Next unanswered item in the selected assessment order. Ask again only about fields marked Unknown, Deferred, or Unconfirmed when the participant is ready to revisit them. Do not infer missing answers from the checkpoint or fill gaps with defaults.
If the scope or spec changed, explain that before continuing and record the change; do not silently reuse answers for items whose meaning or selection changed. A resumed run should produce the same report data as an uninterrupted run when context and confirmed answers are unchanged.
